The chin is in a very prominent position in the face, and thus very much influence facial appearance with the other parts of the face. Deformities related to the chin can either be volumetric or positional or both. Positional abnormalities of the chin may also be the result of the malposition of the mandible or be secondary to maxillary deformity. Many patients with malpositioned, weak, or large chins prefer surgical correction of their chins. Osseous genioplasty with or without orthognathic surgery provides many benefits to these individuals maintaining a well-proportionated and well-positioned look of the chin.

In this lecture the need and timing of genioplasty will be presented along with the surgical techniques and the outcomes and complications will also be discussed.

Synopsis

Orthognathic surgical procedures, stabilized with rigid fixation, are inherently stable. However, in the presence of specific TMJ pathologies, stability will not be predictable with risk of relapse and pain. Most TMJ conditions and pathologies can be placed into specific categories that can aid in determining the nature and expected progression of the TMJ pathology and identify the necessary orthodontic, orthognathic and TMJ surgical protocols to achieve highly predictable outcomes. Common TMJ conditions will be discussed that can cause pretreatment and post-surgical relapse, occlusal instability, dysfunction, and pain. Specific treatment protocols will be presented to simultaneously treat the various TMJ pathologies and correct associated jaw issues (i.e. mal-occlusion, jaw mal-alignment, dysfunction, sleep apnea, pain, etc.) as well as clinical studies to validate these protocols.

Abstract

Orthodontic relapse after combined orthodontic-surgical treatment has the potential to leave our patients unsatisfied and disappointed with their treatment. This lecture will explore the reasons for the relapse and different strategies that can be utilised to reduce the amount of unwanted post-treatment orthodontic changes.

We will explore the effects of dental relapse, skeletal relapse and long-term physiological changes on the alignment of teeth and occlusion. Within this we will consider the effects of periodontal factors, the quality of the final occlusion, impact of soft tissues and patients’ physiological adaptations to surgery, and finally growth, including the timing of surgical treatment and long-term age changes.

We will consider approaches to managing the problem, including the appropriate choice and timing of combined treatment, managing patients’ expectations, and consideration of different types of retainers. When considering the effectiveness of different types of retainers, we will briefly discuss the effectiveness of different types of retainers, survival rates, adverse effects and patient satisfaction with a variety of retainer types used after orthognathic treatment.
